Applying for Financial Assistance

Helping to take the stress out of paying your bills

For more than a century, Norton Healthcare's faith heritage has guided its mission of caring for the sick and injured in the community. That faith heritage influences every aspect of providing health care services to the community - including helping patients make arrangements to pay their bills in compassionate, customer-friendly ways. Some of which include helping patients with and without insurance apply for financial assistance, discount programs and customized payment plans.

As your health care provider, we are concerned not only with your physical well-being, but also with your peace of mind. We understand that making financial arrangements for health care services can be stressful. That's why we want to help.

Norton Healthcare offers two Financial Assistance programs for hospital-based services at the following locations:

  • Norton Hospital
  • Norton Audubon Hospital
  • Norton Brownsboro Hospital
  • Norton Suburban Hospital
  • Kosair Children's Hospital
  • Kosair Children's Medical Center - Brownsboro
  • Norton Diagnostic Center - Dupont
  • Norton Diagnostic Center - Fern Creek

 

Norton Healthcare Financial Assistance

Norton Healthcare Financial Assistance for hospital-based services is available to anyone who meets the eligibility requirements for total gross income, total resources, and then furnishes documentation to confirm this information. Annual income must not exceed 300% of the Federal Poverty guidelines and resources must not exceed 200%.

How to apply for Norton Healthcare Financial Assistance

Required Supporting Documentation

  • Last year's Federal Tax Return
  • 2-3 Recent Pay Stubs of All Earned Household Income
  • Proof of Social Security (Award Letter with Monthly income listed)
  • Proof of Disability Benefits/Income
  • Proof of Pension Benefits
  • Proof of Child Support Benefits/Income
  • Other Income

Mail Financial Assistance Application* and all supporting documentation to:

Disproportionate Share Hospital Program (DSH)

The Disproportionate Share Hospital Program (DSH) is administered by the Commonwealth of Kentucky. As such, DSH income and resource guidelines are set by the Commonwealth of Kentucky. To apply for DSH financial assistance, submit a completed and signed application along with supporting documentation. Because the Commonwealth of Kentucky requires a physical signature, the application must be printed out and mailed, along with supporting documentation. It cannot be completed online.
