We encourage you to learn as much as possible about bariatric weight loss options. The more informed you are, the better your outcome will be. Bariatric surgery is currently approved for people with a body mass index (BMI) of 40 kg/m2 or higher or a BMI of 35 to 39.9 kg/m2 with certain obesity-related medical conditions.
2. Informational seminar
You may attend a seminar at Norton Weight Management Center, 1000 Dupont Road. During the seminar, a member of the Norton Bariatric Center will explain the steps in more detail, and a bariatric surgeon will provide information on adjustable gastric banding and gastric bypass procedures. There also will be question-and-answer time. You do not have to attend a seminar presented by your physician of choice or even at your hospital of choice. 3. Complete patient information packet
Please complete the patient information packet and return it to Norton Weight Management Center. You may fax your packet to (502) 899-6500, drop it by our office at the address below or mail it to:

Completed packet should include:
- Patient questionnaire
- Copies of insurance cards (front and back)
- Medical records (at least past 12 months)
- Letter of support from your primary care physician
- Information from any medically supervised weight management program
Once your completed packet has been received, staff will verify insurance and review your medical records to ensure you are ready to proceed.
4. Norton Weight Management Center assessment
Once your information has been reviewed and your insurance verified, a member of the Norton Bariatric Center will call you to schedule a time for you to come to the office for an assessment. A $300 nonrefundable program fee is due at the time of service. The assessment will include watching a video about surgical options and meeting with a bariatric nurse, dietitian and mental health professional.
5. Surgeon visit
After your assessment, you will be ready to meet with your surgeon. The surgeon will review your medical history, discuss the procedure and answer your questions. Your surgery will be scheduled after approval is received from your insurance company.
6. Pre-admission testing and teaching
Prior to your procedure, you will undergo any necessary testing ordered by your physician. In addition, you will meet with a bariatric nurse and dietitian to make sure you are informed about your procedure and the follow-up care.
Surgeries are performed at Norton Hospital and Norton Women's and Kosair Children's Hospital. Laparoscopic adjustable gastric banding patients spend the evening in the hospital, then complete testing the following day to make sure they are ready to go home. Laparoscopic gastric bypass patients typically spend approximately two days in the hospital. Patients who choose open gastric bypass will be in the hospital approximately four days.
8. Follow-up care
After your surgery, you will follow up with your physician. In addition, the Norton Weight Management Center team is always available for questions. We offer monthly support groups to help you as you adjust to new eating habits and the emotions you are experiencing. The surgery is the easy part – now you must change your lifestyle!
