Orientation Forms

Download the forms on this page, complete them and bring them to orientation. These forms are interactive, which means you can type the information into the forms and then print them. Most of the forms require your signature. Completing these forms before you arrive for orientation will save time, making your first day at Norton Healthcare more enjoyable.

Forms to download, complete and bring to orientation:

W-5 (Earned Income Credit Advance Payment Certificate) – complete this form if you are eligible to get part of the earned income credit (EIC) in advance with your pay and choose to do so. The form includes the information on the eligibility requirements.

I-9 (Employment Eligibility Verification) – complete Section 1, except for your signature; this form must be signed at orientation. Be sure to bring the necessary forms of identification, as described on the form (i.e., driver’s license AND social security card, passport, etc.)

Information Release – states the information about your employment we can release with and without your specific consent.

Identification Information Post Employment – complete the identification information required for government reports.

Parking Form – registers your vehicle and provides you with a parking sticker and card to access the parking garages.

Combined Giving Campaign – provides information on our annual charitable giving campaign; complete the form if you wish to participate for the remainder of the year.

Favorites List – Complete this form to help your manager get to know you – your favorite color, candy, hobbies, etc.
