Geriatric Care Services

Caring for older adults

Geriatric Care Services focuses on caring for the needs of older patients while they are admitted to Norton Audubon Hospital. Our Geriatric Resource Team is made up of specially trained physicians, nurse practitioners, nurses, therapists, pharmacists, dietitians and chaplains. The team provides personalized care for older adults to help them improve their overall comfort and maintain their independence once they are discharged from the hospital. This is accomplished using a model of care designed to maintain or improve patients' strength and stamina with the goal of returning patients to their regular routines as quickly as possible.

During a hospital stay, older patients sometimes experience a general decline in mental and physical health as a result of lack of movement and stimulation. The Geriatric Resource Team works with patients to keep them more active and alert while they receive care for their illness or injuries.

We use a proactive approach to provide specialized inpatient care for older adults and assist them in recovery.

This includes:

  • Coping strategies and emotional/spiritual care
  • Patient and family involvement
  • Safe medication management
  • Physical activity to improve balance and help prevent falls
  • Nutritional counseling
  • Activities for the mind to improve memory and thinking skills
  • Discharge planning/care transition
  • Community education on a variety of topics for caregivers
